To plot or do more!

Hi all

On your MEWS charts - do you just plot the BP or do you plot the BP and document  numerically above the plotted recording?

Does anyone have any articles/research findings that discusses the most effective way in recording observation findings onto the MEWS chart so trends are not missed?

I've started a lit search but keen to hear from others

Our Clinical Emergency Response Co-ordinator preaches to us to only plot - no numericals. It does make for an easier to read trend. She often cites the Clinical Excellence Commission and the Australian Resusitation Guidelines, so try them,

There should only be plotting as per the National Standards.
